Position Overview
LTSS Service Coordinator - Case Manager (Region
F: Lee/Sarasota County) Bilingual in Spanish/English. Monday‑Friday 8am–5pmEST. Field‑based; must reside in Charlotte, Collier, Desoto, Hendry, Glades, Lee, or Sarasota County (commuter distance to office may be considered).

Responsibilities- Conduct service coordination for a defined caseload of individuals in specialized long‑term services and supports (LTSS) programs.
- Facilitate Person‑Center Planning (P‑CPlanning) including assessments, development of a comprehensive Person‑Center Support Plan (PCSP), and backup plan.
- Interface with Medical Directors, participate in interdisciplinary care rounds, and coordinate care plans that integrate physical, behavioral and LTSS needs.
- Perform face‑to‑face program assessments using pre‑defined tools and motivational‑interviewing techniques to identify clinical and non‑clinical needs.
- Serve as the single point of contact for members identified with high‑risk complications or gaps in care, coordinating with the clinical healthcare management and interdisciplinary team.
- Manage non‑clinical needs for members with chronic illnesses, co‑morbidities, and/or disabilities to ensure cost‑effective utilization of services.
- Document member goals and collaborate with caregivers, family, natural supports, service providers, and physicians.
- Identify members who may benefit from alternative service levels or other waiver programs.
- Present as a mentor, subject‑matter expert, or preceptor for new staff and assist with formal training.
- Participate in process‑improvement initiatives.
- Submit utilization/authorization requests with supporting documentation.
- Report critical incidents to appropriate internal and external parties (e.g., Adult Protective Services, law enforcement).
- Assist in appeals, fair hearings, member grievances, and state audits.
- BA/BS degree and minimum of 2years of experience working with a social‑work agency (or a combination of education and experience providing an equivalent background).
- Bilingual in English and Spanish strongly preferred.
- Experience in LTSS, case management, social work, or hospital discharge planning preferred.
- Strong communication, interdisciplinary collaboration, and motivational‑interviewing skills.
