Position: Peer Family Navigator (50169)
Position Summary:

The Family Navigator helps families achieve stability by providing guidance, resources, and emotional support. They collaborate with parents, foster families, and case managers to ease transitions and strengthen family bonds. Responsibilities include assisting with case management, tracking referrals, and coordinating services. This role also advocates for families, ensuring they receive the support they need.

Essential Job Functions:
  • Support the well-being, independence, and self-sufficiency of clients while guiding them through case plans to encourage successful reunification.
  • Collaborate with parents, foster parents, and case managers to create a smooth and supportive transition plan for children returning home.
  • Assist with training team members and provide support as needed.
  • Foster positive relationships with parents to strengthen family bonds, enhance parent-child connections, and encourage engagement in case planning and services.
  • Offer guidance, education, and supportive services tailored to the needs outlined in each service plan.
  • Work alongside case managers, counselors, and social workers to provide case management support, including information sharing, assessments, referrals, and connections to resources.
  • Track referrals and service linkages to ensure consistency and continuity of care.
  • Encourage and empower clients by sharing experiences, offering emotional support, and helping them access the services and tools they need to succeed.
  • Arrange or provide transportation for clients to visitations, medical appointments, counseling sessions, or other essential activities.
  • Assist with scheduled visits, in-home services, and other supportive activities as needed.
  • Participate in weekly staff meetings to discuss client progress and provide input as requested.
  • Support the team and organization with additional responsibilities as needed.
  • Advocate for and support children and families in a caring and respectful manner.
  • Serve as a family/individual representative and advocate in program/agency activities.
  • Assist in the facilitation of individual/family support and educational groups.
  • Provide Peer support to individuals/families by using personal and professional life experiences.
  • All other duties as assigned.
Essential Qualifications:
Certifications/Licensures:
  • Certified Recover Peer Specialists or Certified Recovery Support Specialist through the Florida Certification Board must be obtained within one (1) year of hire; preference will be given to candidates who are currently certified.
Education:
  • Training and/or experience in Child Welfare is preferred.
  • Two (2) years demonstrated recovery time from a significant mental health and/or substance abuse disorder at the date of application and within the last year either maintained at least 12 months of successful work or volunteer experience.
  • High School Diploma or equivalent.
Experience and

Competencies:
  • Bilingual is preferred.
  • Physical required prior to hire; paid by agency
  • Expertise with the Florida Child Welfare Practice Model and knowledgeable in Child Welfare electronic system of record (FSFN or CCWIS).
  • Possess strong leadership, communication, problem-solving, and decision-making skills.
  • Availability to work a flexible schedule as required by the department, including nights and weekends.
  • Possess a current Background Clearance Screening Letter as required by DCF.
  • Valid Florida driver's license and ability to obtain and maintain West Care's vehicle insurance is required;
  • The ability to identify and evaluate an individual's strengths, barriers, problems, and needs for the development of a Wellness and Recovery Plan
  • The ability to write assessments, progress notes, discharge summaries, and other consumer related data
  • Knowledge of the inter-relationship among substance use, mental health, and primary care disorders
  • Knowledge and application of developmental theories
  • The ability to evaluate the impact of substance use and mental health problems on the consumer's functioning in major life area
