Job Title

LTSS Service Coordinator – Case Manager (Region E: Brevard/Orange/Osceola/Seminole County)

Location & Field

This is primarily a field‑based position. Candidates must reside in Brevard, Orange, Osceola, or Seminole County. Travel to client sites is required; occasional office attendance for meetings or training is expected.

Schedule

Monday‑Friday, 8:00am – 5:00pm EST

Role Summary

The LTSS Service Coordinator manages a defined caseload of individuals in specialized long‑term services and supports programs. The role involves conducting assessments, developing Person Centered Support Plans, coordinating with interdisciplinary care teams, and overseeing the physical and behavioral health needs of clients.

Responsibilities
  • Perform face‑to‑face program assessments using standardized tools
  • Use motivational interviewing techniques to evaluate and address clients’ needs
  • Coordinate and manage waiver eligibility and service utilization (e.g., LTSS/IDD, BH, PH)
  • Serve as the single point of contact for clinical healthcare coordination and interdisciplinary team collaboration
  • Identify gaps in care and potential health risks; coordinate necessary services
  • Document short‑ and long‑term service goals in partnership with clients and care teams, including family and providers
  • Identify opportunities for alternative service levels or other waiver programs
  • Assist in training and mentoring new staff; contribute to process improvement initiatives
  • Submit utilization and authorization requests with supporting documentation
  • Report critical incidents to state and county agencies and participate in appeals, grievances, and audits
Minimum Requirements
  • BA/BS degree
  • Minimum of 2years of experience working with a social work agency (or equivalent education/experience)
  • Knowledge of state law and contract requirements (as applicable)
Preferred Skills, Capabilities and Experience
  • Bilingual Spanish/English (strong preference)
  • Experience with LTSS, case management, social work, or hospital discharge planning
  • Background in health‑care related fields
