About the Role

The Children Care Coordinator plays a pivotal role in ensuring that children receive comprehensive and effective health care services tailored to their unique needs. This position involves collaborating with families, healthcare providers, and community resources to create and implement individualized care plans. The coordinator will monitor the progress of each child, making necessary adjustments to care strategies to optimize outcomes. By fostering strong relationships with families and advocating for their needs, the coordinator helps to bridge gaps in care and support.

Ultimately, the goal is to enhance the overall well‑being and development of children within the behavioral health framework.

Minimum Qualifications
  • Bachelor's degree in social work, psychology, nursing, or a related field.
  • Experience working with children in a healthcare or behavioral health setting.
  • Strong communication and interpersonal skills.
Preferred Qualifications
  • Master's degree in social work, psychology, or a related field.
  • Certification in care coordination or case management.
  • Experience with community outreach and resource navigation.
Responsibilities
  • Develop and maintain individualized care plans for children in collaboration with families and healthcare professionals.
  • Coordinate and facilitate communication between families, healthcare providers, and community resources to ensure comprehensive care.
  • Monitor and evaluate the progress of children, making adjustments to care plans as necessary.
  • Provide support and guidance to families, helping them navigate the healthcare system and access available resources.
  • Conduct regular follow-ups with families and providers to assess the effectiveness of care strategies and make recommendations for improvement.
