LTSS Service Coordinator – Case Manager (Region C: Pasco/Pinellas)

Anticipated End Date:

Field‑based position. Candidate must reside in Pasco or Pinellas County.

Monday‑Friday, 8:00am–5:00pm EST

• Conduct service coordination for a defined caseload of individuals in specialized programs.
• Facilitate the Person Centered Planning process: assessments, development of a comprehensive Person Centered Support Plan (PCSP) and backup plan, and backup plan.
• Interface with Medical Directors and participate in interdisciplinary care rounds to develop a fully integrated care plan.
• Engage the member’s circle of support and manage physical, behavioral, and long‑term services and supports needs as required by state law, contract, and federal requirements.
• Perform face‑to‑face program assessments using tools with predefined questions; apply motivational interviewing techniques.
• Identify members with potential clinical health care needs, coordinate their cases with the interdisciplinary team, and serve as the single point of contact.
• Manage non‑clinical needs of members with chronic illnesses, comorbidities, or disabilities to ensure cost‑effective and efficient utilization of services.
• Document short‑ and long‑term service and support goals in collaboration with the member’s care team.
• Identify members that may benefit from alternative levels of service or other waiver programs.
• Serve as mentor, subject matter expert, or preceptor for new staff and assist in formal training of associates.
• Submit utilization/authorization requests with documentation supporting and aligning with the individual’s care plan.
• Report critical incidents to appropriate internal and external parties (e.g., Adult Protective Services, Law Enforcement).
• Assist and participate in appeals, hearings, grievances, and state audits.

• Bachelor’s degree (BA/BS) and a minimum of 2years of experience working with a social work agency; or any combination of education and experience that provides an equivalent background.
• Knowledge of state law and contract requirements related to long‑term services and supports.

• Bachelor’s degree in a health‑care related field.
• Bilingual in English and Spanish.
• Experience in LTSS, case management, social work, or hospital discharge planning.
