Schedule:

Monday – Friday, 8am – 5pm EST. Must be open to travel up to 2 hours daily and reside in one of the designated counties. This is a field‑based role with occasional office attendance for meetings or training.

• Assist management with day‑to‑day activities, including monitoring and prioritizing workflow.
• Review and ensure forms are completed accurately and on schedule.
• Develop and maintain policies, procedures, training materials, and job resources.
• Lead projects, participate in work groups with cross‑functional teams, and serve as a representative on business unit and enterprise initiatives.
• Perform process evaluations, tests, and monitor systems/process enhancements.
• Manage a member’s individual service plan within the scope of the position, ensuring it meets state law and contract requirements.
• Develop, monitor, and revise service plans to optimize member health care across the care continuum.
• Conduct telephonic or face‑to‑face history and program needs assessments using a defined tool.
• Identify members with potential clinical health care needs and coordinate these cases with the clinical healthcare management and interdisciplinary team.
• Manage non‑clinical needs of members with chronic illnesses, co‑morbidities, or disabilities to ensure cost‑effective and efficient utilization of long‑term services.
• Establish short and long‑term service and support goals with members, caregivers, family, natural supports, and physicians.
• Identify members who could benefit from alternative levels of service or other waiver programs.
• Serve as mentor or preceptor for new staff, assist in formal training, and participate in process improvement initiatives.

• Bachelor’s degree (BA/BS) and a minimum of 2 years of experience working with a social work agency.
• At least 5 years of experience working with individuals with chronic illnesses, co‑morbidities, and/or disabilities in a Service Coordinator or similar role, or an equivalent combination of education and experience.

• Bilingual in Spanish/English.
• BA/BS degree with a healthcare‑related field of study (preferred).
• Willingness to travel to worksites and other locations as necessary.
