Center for Autism and Related Disabilities (CARD) has an open Case Manager position to work with adolescents and adults with autism and related disabilities. Bilingual (English/Spanish). Bachelor's degree in special education, psychology, behavior analysis, social work or related field and 3 to 5 years of relevant work experience required. Excellent interpersonal communication skills, compassion, time management, organization, and problem-solving skills are a must.

Expected to have good public speaking skills, and a dynamic presentation style. This position is based in Miami-Dade County.

The Case Manager coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. The incumbent also provides ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient’s needs. An employee in this position acts as a liaison between patients, families, and healthcare personnel to ensure necessary care is provided promptly and effectively.  
** CORE JOB FUNCTIONS
*** Coordinates the integration of the social services function into patient care.
* Assists patients in understanding their conditions and possible medical treatments.
* Works with affiliated agencies and acts as an advocate between service recipients and these agencies.
* Designs health plans for patients, which contain several activities from administering medication to consulting with other healthcare personnel.
* Collaborates with other professionals to evaluate patients' medical or physical conditions and relevant needs.
* Identifies environmental impediments to client or patient progress through interviews with patients and review of patient records.
* Monitors, evaluates, and records client progress per measurable goals described in treatment and care plan.
* Utilizes consultation data and social work experience to plan and coordinate patient care and rehabilitation, following through to ensure service efficacy.
* Adheres to University and unit-level policies and procedures and safeguards University assets.
** Department Specific Responsibilities:
*** Provides direct individual family assistance to all eligible adolescent and adult CARD clients. Consistently exercises discretion and judgment in analyzing, interpreting, and determining a proper course of action, support, and resource referrals that best serve client needs. Works closely with the client/family, demonstrating good judgment and discretion to ensure that referrals are appropriate for the changing client needs across the lifespan.
* Maintains an active caseload of several hundred adolescent and adult CARD clients, independently providing an array of educational, transition, and employment support services in schools, homes and other community settings. Facilitates case coordination for these clients and information sharing with other providers involved in the case (as permitted by law).
* Provides expert consultation and technical assistance services to local schools and other educational programs serving individuals with autism and related disabilities.
* Develops and conducts highly specialized constituent/family and professional training, workshops, seminars, and conferences on topics related to the education, transition services, employment, treatment, and better understanding of autism and related disabilities. Expected to have good public speaking skills, strong organizational skills, and a dynamic presentation style.
