Lutheran Services Florida seeks a SNAP Case Manager to support individuals and families in accessing food assistance. This role provides intake, eligibility screening, application support, and ongoing case management for SNAP participants. The Case Manager offers outreach, education, and advocacy, connecting clients to additional social services as needed. Responsibilities include detailed documentation, data entry, and compliance with state and federal regulations.

Ideal candidates are organized, compassionate, and skilled in client communication within diverse, vulnerable communities.

Responsibilities

  • Conduct eligibility screenings and applications for SNAP benefits with individuals and families.
  • Provide case management, including intake, assessment, service planning, and follow-up.
  • Educate clients on SNAP program rules, documentation requirements, and recertification timelines.
  • Maintain accurate, timely case notes and data entry in electronic case management systems.
  • Coordinate with internal programs and community partners to connect clients to additional resources.
  • Monitor case progress, address barriers to benefits access, and advocate on behalf of clients.
  • Ensure compliance with all federal, state, and agency SNAP policies and confidentiality standards.
  • Participate in outreach events, presentations, and workshops to increase SNAP program awareness.
  • Collaborate with a multidisciplinary team to support holistic client and family needs.
  • Prepare reports and outcome summaries for supervisors and funding or regulatory bodies.
