We are seeking a Social Services Coordinator to help older adults and their families find the programs and services they need.
In this role, you will speak with clients and family members, understand their needs, screen and prioritize requests, and connect them with appropriate community resources. You will also document client information and follow up on referrals.
What You'll Do- Speak with older adults and family members by phone
- Ask questions to understand their needs and circumstances
- Screen and prioritize requests based on urgency
- Provide information about available programs and services
- Connect clients with appropriate community resources
- Document calls, referrals, and client information accurately
- Follow established procedures and maintain timely follow-up
- Provide general support to the department as needed
- 2+ years of experience in social work, human services, or a related field
- Strong communication and interpersonal skills
- Comfortable working with older adults and their families
- Strong computer skills, including Microsoft Word and Excel
- Typing speed of 45+ WPM
- Compassionate, patient, and professional communication style
- Strong attention to detail and organization
- Hybrid work environment
