You are a compassionate, resourceful, and client-centered professional with experience supporting individuals facing complex challenges, including homelessness, mental health conditions, and substance use disorders. You excel at building trust, navigating systems of care, and coordinating services in a culturally sensitive and trauma-informed manner. You thrive in collaborative, multidisciplinary environments and balance empathy with accountability, ensuring clients are empowered to achieve their housing, income, and wellness goals.

What

You’ll Do

As a Case Manager, you will provide intensive case management and supportive services to individuals in our residential treatment program who are experiencing homelessness and co‑occurring disorders. You will help clients identify barriers, connect with resources, and work toward housing stability, wellness, and self‑sufficiency.

Key Responsibilities
  • Conduct client assessments to identify needs, strengths, and barriers related to housing, income, wellness, and independent living.
  • Develop and update individualized service plans with clear and measurable goals.
  • Meet with clients regularly to review progress, address challenges, and provide ongoing support.
  • Connect clients with housing, benefits, healthcare, behavioral health, employment, and other community resources.
  • Coordinate services and advocate for clients with internal teams and outside providers.
  • Participate in case conferences, team meetings, case staffing, and community partner meetings.
  • Respond to urgent client needs and provide appropriate follow‑up support.
  • Assist with housing placement, transition planning, discharge planning, and follow‑up services.
  • Maintain accurate and timely case notes, client files, reports, and service documentation.
  • Enter client information into HMIS and other required databases.
  • Follow all agency policies, contractual requirements, and confidentiality standards.
  • Build positive working relationships with community partners and service providers.
  • Attend staff meetings, trainings, and supervision sessions.
  • Support special projects and other departmental needs as assigned.
  • Perform other related duties as assigned.
Requirements

What You’ll Bring
  • Education:

    Bachelor’s degree in Social Work, Psychology, Human Services, or related field (equivalent experience may be considered).
  • Experience:

    At least 2 years of case management, behavioral health, or social services experience, preferably with individuals experiencing homelessness and/or co‑occurring disorders.
  • Skills & Abilities:
  • Strong organizational skills with the ability to manage multiple priorities.
  • Excellent interpersonal and communication skills, both written and verbal.
  • Cultural sensitivity and the ability to work effectively with diverse and vulnerable populations.
