The Targeted Case Manager is a full‑time, on‑site role based in Miami, FL. The person in this role will coordinate and monitor services for clients with behavioral health needs, including assessing client needs, developing individualized service plans, and connecting clients to appropriate community resources. Day‑to‑day tasks include maintaining regular contact with clients, advocating for services, collaborating with clinicians and external providers, and documenting all case management activities in accordance with regulatory and clinic standards.

The role also involves crisis support coordination, follow‑up on referrals, and ensuring that services remain aligned with each client’s goals. The Targeted Case Manager works closely with a multidisciplinary team to promote continuity of care and positive client outcomes.

• Experience in case management, social services, or behavioral health coordination, with the ability to assess needs and develop individualized service plans.
• Strong communication and interpersonal skills to build rapport with clients, families, and community partners, including advocacy and collaboration.
• Proficiency in documentation, record‑keeping, and time management to maintain accurate client files and meet deadlines and compliance requirements.
• Knowledge of community resources, mental health services, and support programs relevant to clients in the Miami, FL area.
• Ability to work on‑site, manage a caseload, and navigate complex systems while maintaining professional boundaries and confidentiality.
• Bachelor’s degree in social work, psychology, human services, or a related field preferred; relevant experience in lieu of degree may be considered.
• Familiarity with Medicaid or other managed care requirements, and understanding of behavioral health regulations and best practices.
• Bilingual English/Spanish skills are highly beneficial, given the local community served.
