Key Responsibilities
  • Conduct medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment.
  • Performs independently under stress and in emergency situations.
  • Drive between locations, navigating to each location using GPS device or other tools.
  • Communicate significant findings, problems, and changes in condition or environment to the clinical coordinator, family, facility and other personnel involved with patient care.
  • Assess health status by obtaining relevant health and medical history. Perform a physical exam based upon the age and patient history. Identify significant health and medical risk factors.
  • Order, interpret and evaluate laboratory tests and other diagnostic studies to assess patient’s clinical problems and health care needs. Develop a diagnosis by analyzing and synthesizing collected data.
  • Perform diagnostic and therapeutic interventions consistent with certification, law and state medical privileges. May prescribe and monitor medications as allowed by prescriptive authority through the appropriate State Board Agency.
  • Develop a treatment plan in collaboration with the patient and significant others to include Medicare, CMS, ancillary services, and treatment. Collaborate with nursing, medical and other healthcare team members to ensure quality patient care.
  • Refer patients to providers as appropriate for consultation or for specialized health resources and treatment. Ensure continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas as necessary.
  • Supervise nurse practitioners and physician assistants per the Collaborative Agreement.
Required Qualifications
  • Doctor of Medicine or Osteopathy required.
  • Current unrestricted state license to practice medicine.
  • Prior supervisor / management experience.
  • Demonstrated interpersonal, supervisory, and management skills with ability to communicate effectively.
  • Proven organizational development and project management skills.
  • Working knowledge of regulations and reimbursement in home health care plus proven leadership abilities.
  • Physical ability to sit, stand, bend, lift, and move intermittently; able to lift at least 50 lbs and bear the weight of an average adult.
  • Current and valid driver's license; reliable transportation and valid automobile insurance coverage.
Desired Qualifications
  • Board certification in family practice, internal medicine, geriatric or palliative care preferred.
  • Preferred 2 years experience in healthcare, with a combination of acute care, geriatric and/or long‑term care practice.
  • Experience with geriatric patients preferred.
Location

Home Office: 24255 W 13 Mile Rd STE 210, Bingham Farms, MI 48025.

Service Area

Bingham Farms and surrounding areas. This position services patients in facilities and private home settings.

Hours

Office hours: 8:00 AM – 5:00 PM, Monday through Friday.

Pay Range

$150,000—$200,000 USD.
