Provide comprehensive outpatient primary care to adult and family medicine patients Diagnose, treat, and manage acute and chronic conditions Conduct preventive care visits and wellness exams Develop and coordinate long-term patient treatment plans Document accurately using EPIC EMR Collaborate with multidisciplinary teams to optimize patient outcomes Participate in shared on-call duties as assigned Support patient education and continuity of care Engage in quality improvement initiatives Build strong relationships within the community What You Need:
Michigan medical license (or eligible to obtain) Board Certified in Family Medicine or Internal Medicine (must not have lapsed) Ability to work independently and within a team Excellent communication and patient care skills Willingness to work in a community-focused outpatient setting About the Assignment:
