- Provide comprehensive outpatient primary care to adult and/or family medicine patients
- Diagnose, treat, and manage both acute and chronic conditions
- Conduct preventive care visits, wellness exams, and follow-ups
- Develop and coordinate long-term treatment plans with specialists as needed
- Document all clinical encounters accurately using EPIC EMR
- Collaborate with multidisciplinary teams to enhance patient outcomes
- Participate in shared on-call duties (as assigned)
- Support quality improvement initiatives and practice in accordance with organizational standards
- Build trusted relationships with community members and support continuity of care
- Active Michigan Medical License
- Board Certified in Family Medicine or Internal Medicine (not open to providers needing to re-credential)
- Minimum of 0+ years’ experience (new graduates welcome)
- Strong clinical and communication skills
- Ability to work collaboratively in a team-based outpatient setting
