Job Summary
The Alumni Liaison plays a vital role in supporting clients as they transition out of programming and continue their recovery journey. This position is responsible for maintaining ongoing communication with alumni, collecting outcome data through assessments, and ensuring alumni remain connected to the behavioral health campus. The Alumni Liaison also provides limited case management during active programming to prepare clients for discharge, connect them with outpatient providers, maintain relationships with outpatient providers, and encourage participation in the full continuum of care.
As part of the sales funnel and growth team, the Alumni Liaison monitors key performance indicators (KPIs) related to step-downs, discharges to outpatient providers, alumni engagement and re-entry, ensuring that clients are stepped up into higher levels of care when clinically appropriate and that program growth goals are met.
Role and Responsibilities
- Develop and grow an alumni ambassador program where clients share their recovery and/or healing stories.
- Implement and maintain structured communication with alumni post-discharge to foster ongoing connection and support.
- Ability to build rapport with clients and alumni, fostering trust and long-term engagement.
- Distribute and track alumni assessments to collect data on outcomes, satisfaction, and ongoing needs.
- Monitor KPIs related to alumni re-engagement, including stepping clients up into higher levels of care when needed.
- Meet with clients during programming to begin discharge planning and establish continuity of care.
- Assist clients in connecting with outpatient providers and coordinate with current providers when applicable.
- Educate clients on the benefits of engaging in the full continuum of care.
- Collaborate with the sales and growth team to align alumni engagement strategies with organizational goals.
- Track and report alumni engagement metrics, re-entry rates, and client progression through the continuum of care.
- Provide insights and recommendations to leadership based on data trends and alumni feedback.
- Partner with clinical, case management, and outreach teams to deliver consistent, high-quality support.
- Comply with organizational policies and procedures, including code of conduct and policies resulting from the HIPAA privacy rules regarding PHI concerning persons serviced or employed.
- Coordinate with clients to develop a discharge plan and continuing care plan.
- Facilitate assessments post-discharge.
- Communicate with outpatient providers or external programs regularly through the client’s treatment episode to provide updates and assist with a smooth transition of care.
- Facilitates ongoing communication with alumni to collect data, connect them to resources, organize meetings, and keep clients and families connected to the recovery community.
- Performs other duties as assigned.
Qualifications and Education Requirements
- Bachelor’s degree in human services related field (i.e., Social Work, Psychology, Sociology), preferred.
- Prior experience working in behavioral health, substance use treatment, or mental health services (required).
- Possession of a valid driver’s license.
- Two (2) years prior Case Manager experience, preferred.
Knowledge/Skills/Abilities
- Strong organizational skills with experience in person-centered planning.
- Demonstrated understanding of persons with co-occurring mental health disorders, substance use disorders, and/or eating disorders.
- Demonstrated communication skills that effectively convey information.
- Demonstrated active listening (i.e., listening to what other people say and asking appropriate questions).
- Knowledge of trauma-informed theories, principles, and practices (includes multi-faceted understanding of concepts such as community trauma, intergenerational and historical trauma, parallel processes, and universal precautions).
- Demonstrated leadership skills and good decision-making ability.
- Knowledge of case management pre and post discharge.
- Ability to collaborate with a team milieu approach to the treatment of clients.
