, you’ll play a vital role in supporting residents’ emotional well‑being, care planning, and successful transitions while advocating for dignity, respect, and quality of life.

What You’ll Do- Develop, plan, implement, and evaluate the Social Services program in accordance with federal, state, and center guidelines
- Identify and address residents’ social, emotional, and psychological needs
- Coordinate services to support resident adjustment, care planning, and discharge needs
- Collaborate with interdisciplinary teams to ensure holistic, person‑centered care
- Assist residents and families with resources, education, and care transitions
- Maintain accurate documentation in compliance with regulatory standards, including MDS‑related processes
- Advocate for Residents’ Rights and ensure concerns are addressed promptly and compassionately
- Support compliance with Medicare, Medicaid, and other reimbursement and eligibility guidelines
- Licensed Social Worker (required)
- At least one year of supervised social work experience in a healthcare setting, preferably working with geriatric populations
- Working knowledge of nursing home regulations, MDS procedures, and reimbursement guidelines
- Strong written and verbal communication skills
- Compassionate, service‑oriented mindset with a desire to support residents and families
- Positive, dependable, and team‑focused attitude
