We are hiring a Physical Therapist (PT) for a Home Health position providing skilled care to patients in their homes in the McBain and surrounding areas and is eligible for $20,000 total bonus potential, including a $10,000 first year success bonus and an additional $10,000 sign‑on bonus. WhatYou’ll Do
- Deliver skilled, patient‑centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
- Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence.
- Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home.
- Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
- Work independently, but never alone.
- Travel to patient homes within assigned territory.

- Active Physical Therapist, PT license in the state you work.
- One year of experience as a Physical Therapist and clinical expertise to deliver quality patient care.
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
- Successful completion of a drug screen prior to employment (includes medical and recreational marijuana).
- You are legally authorized to work in the United States.
