Certified Social Worker (Home Health)

Job Description:

• Conduct psychosocial assessments of patients and families
• Develop and implement individualized care plans
• Provide counseling, crisis intervention, and emotional support
• Coordinate community resources and support services
• Assist with Medicaid, disability, and other assistance program applications
• Participate in interdisciplinary care team meetings
• Maintain accurate and timely documentation
• Ensure compliance with Medicare, Medicaid, and agency guidelines

Requirement:

• Bachelor's or Master's degree in Social Work (MSW preferred)
• Current state Social Worker license/certification
• 1–2 years of social work experience (Home Health, Hospice, or Healthcare preferred)
• Knowledge of community resources and healthcare services
• Valid driver's license and reliable transportation
• Strong communication, counseling, and documentation skills

Preferred Qualifications:

• Home Health or Hospice experience
• MSW degree
• Knowledge of Medicare/Medicaid regulations
