• Flexible Patient-Care Hours :
  You will have the flexibility to build your own schedule within our operating hours of 7:00 am to 6:00 pm, Monday through Friday. You are required to provide a minimum of 36 hours of patient care per week, either face-to-face or through telemedicine, with additional hours for completing medical records and other non-direct patient responsibilities.

You will have the opportunity to conduct diagnostic evaluations and offer individual and group therapy sessions. You'll employ therapeutic techniques such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing, and other methods tailored to your patients' needs. Specializing in conditions like PTSD and substance use disorder, you'll create and implement personalized treatment plans. You'll provide counseling services to both adults and children, in person and via telehealth platforms.

By your fourth month of employment, you will be expected to maintain a schedule of 40 patients per week (460 quarterly, 1840 annually). We regularly review patient care volumes to ensure you are supported in meeting these goals.

Upper Great Lakes Family Health is your community-based medical, dental, and behavioral health care center. We are designated as a Federally Qualified Health Center (FQHC), providing services regardless of language, gender, socio-economic status, sexual orientation, physical and mental capacity, age, religion, housing status and ability to pay.

• Licensed Master Social Worker (LMSW), Limited Licensed Master Social Worker (LLMSW) or Licensed Professional Counselor (LPC).
• Evidence of basic life support (BLS) certification or willingness to obtain upon hire.
• Preferred Experience:
  
  Two years’ experience in the behavioral health field.
