Position: Speech Language Pathologist I
Location: Tawas City

Summary

Speech-Language Pathologist provides professional and comprehensive care. Activities of this position are patient focused and utilize assessment, planning, intervention, and evaluation in accordance with physical therapy standards, physician orders, and governing policies and procedures.

This position will be split between Tawas and Standish locations, with the majority of time in Tawas.

Responsibilities
  • (25%) Evaluates the patient’s function by performing formal/informal testing measures to determine the skeletal, neurological, respiratory, cardiovascular status, developmental, social and cognitive functions.
  • (25%) Develops a goal specific treatment regime based on assessment data and the patient’s tolerance to carry out the treatment plan as prescribed.
  • (25%) Submits a written treatment regime to be included as part of the plan of treatment authorized by and coordinated with the physician. Updates plan of care as needed to reflect changes in patient status.
  • (25%) Devises a schedule of treatment, which is frequency and duration specific to be discussed with patient/family/caregiver and coordinated with clinical support staff.
Other Duties and Responsibilities
  • Educates patient, family, and/or caregiver regarding pathology or condition, patient alternatives, prognosis, and rationale for treatment through forms of rest, exercise, modalities, and other indicated interventions.
  • Provides supervision for CFY‑Speech Language Pathologist, Home Health Aides and students per affiliate and regulatory standards.
  • Recommends adaptive/supportive equipment based on assessment; contacts appropriate representative and/or supplier to place order and assists in securing equipment for patient.
  • Documents the patient/family/caregiver response to treatment and changes in patient condition per affiliate documentation policies and or standards and in compliance with regulatory agencies.
  • Completes daily documentation necessary for billing procedures per affiliate policy and in compliance with regulatory agencies.
  • Provides treatment intervention and demonstrates competence to facilitate or restore optimal functioning specific to therapist’s patient care area.
  • Educates patient/family/caregiver in home education program using relevant and effective teaching methods.
  • Demonstrates effective oral and written communication with all internal and external customers to maximize quality of patient care including coordination with interdisciplinary team members.
  • Promotes education and development among staff, students, and other health care providers using relevant and effective teaching methods. Promotes process improvement. Assists with program development.
  • Participates in team conferences and interdisciplinary meetings as appropriate.
  • Promotes speech language pathology within My Michigan Health and service area, participating in and providing community education.
  • Functions as case manager for assigned cases.
  • Demonstrates commitment to My Michigan Health and specific affiliate mission as evidenced by customer service, quality of service, and teamwork.
  • Provides speech language pathology services to My Michigan Affiliates as needed to assist with patient coverage for continuity of patient care.
  • Other duties as assigned.

Certifications and Licensures
  • Credential: SLP CCC: SLP Cert Clinical Competence
  • Equivalent

    Experience:

    Current Certificate of Clinical Competence granted by the American Speech, Language, Hearing Association within 1 year of hire or transfer. Licensure in the State of Michigan required effective 12/07/2013. May consider a candidate who meets educational requirements for certification and is in the process of accumulating the supervised experience required for certification. ASHA membership required.
