Senior Solutions – MSW

Job Purpose

The Senior Life Solutions Program Therapist provides therapeutic services to older adults in an intensive outpatient environment, delivering group, individual, and family therapy sessions. The role operates within a small interdisciplinary team focused on quality patient care and the provision of services ordered by a physician. Documentation of all completed services must comply with established standards and regulations.

• Ensure accurate completion of the psychosocial assessment on each patient.
• Ensure accurate completion of group psychotherapy notes for each patient.
• Ensure accurate completion of treatment plans.
• Demonstrate understanding of mental health assessment of older adults.
• Demonstrate understanding of gerontological counseling treatment, group therapy, theory, and evidence‑based practices.
• Provide understanding of gerontological case management.
• Ensure understanding of mental disorders associated with aging.

• Participate in outcome studies timelines, understanding the administration of the tests and when they are to be reported.
• Participate in treatment team meetings with the medical director.
• Comply with the 25‑point checklist.
• Complete all notes in the prescribed timeline.
• Comply with all regulations.

• Demonstrate understanding of community education definitions and activities.
• Participate in customer service training as it relates to community integration.
• Participate in a cohesive team environment to ensure program success.

• Education:
  
  Master’s degree in social work from an accredited graduate school of social work or master’s degree in another counseling‑related field.
• Licensure:
  License in Social Work or Counseling as recognized by the state (e.g., LCSW, LPC, LMFT, etc.).
• Certification:
  Must meet any applicable state certification standards.
• Experience:
  
  Two to three years of clinical experience with older persons, extensive group work, and hospital or nursing home experience preferred.

Incumbent will be exposed to viruses, diseases, and infections from patients.

Award regularly required to talk and hear; specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. The position requires intermittent physical activity, including standing, walking, bending, kneeling, stooping, crouching, lifting, driving, and supporting patients.

This position has no direct reports.
