Provide skilled Physical Therapy services on an intermittent basis to patients in their homes by physician orders.
- Conduct initial and ongoing comprehensive assessments of patient needs, including OASIS assessments.
- Establish and revise treatment plans in consultation with physicians.
- Assist in developing and implementing interdisciplinary patient care plans related to Physical Therapy.
- Conduct diagnostic and prognostic muscle, nerve, joint, and functional ability tests.
- Treat patients to relieve pain, restore function, and maintain performance using exercise, massage, heat, water, light, and electricity.
- Consult and familiarize staff regarding established plans.
- Teach, supervise, and counsel patients and families in the physical therapy program.
- Instruct patients in the care and use of wheelchairs, braces, crutches, canes, and prosthetic and orthotic devices.
- Provide necessary equipment to carry out the plan.
- Arrange for outpatient services for procedures that cannot be given at home.
- Record evaluation data, treatments, and patient responses in a timely manner; also record quality assurance data.
- Prepare assessments, periodic evaluations, and summaries.
- Supervise Physical Therapy Assistants and paraprofessionals as per agency policy.
- Report patient reactions to treatment and any changes in condition to physicians.
- Communicate with physicians at least every 30 days or whenever changes occur regarding patient status and obtain additional orders if necessary.
- Communicate with team members, including Home Health Aides, and instruct them in applicable phases of physical therapy.
- Write clinical/progress notes on the date of the visit and incorporate them into the patient record within 14 days.
- Participate in case conferences, staff meetings, agency activities, and in‑service programs as needed.
- Supervise home health aides every two weeks when physical therapy is the sole professional discipline involved and supervise PT assistants at least monthly.
- Participate in discharge planning activities and complete a Physical Therapy Discharge Summary.
- Obtain a medical history, particularly as it relates to the current condition.
- Evaluate patients, significant others, and home situations to determine necessary instruction, family assistance, and required community services.
- Ensure physician orders are appropriate and discuss necessary changes.
- Be able to drive and access patient homes that may not be routinely wheelchair accessible.
- Maintain sufficient hearing, eyesight, and physical dexterity to perform assessments and patient care.
- Be able to bend, stoop, reach, and move patient weight up to 250 pounds; lift and/or carry up to 30 pounds as required.
- Communicate effectively in English.
- Maintain patient confidentiality, including all HIPAA regulations.
