STARTING WAGE RANGE: $24.64 - $25.88 per hour

QUALIFICATIONS

Education:

Minimum of a bachelor’s degree in social work or other relevant human service required, with licensure/limited licensure as appropriate from State of Michigan;
Must be either a Qualified Intellectual Disability Professional (QIDP) or Qualified Mental Health Professional (QMHP)

Qualified Intellectual Disability Professional (QIDP) – Individual with specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons with intellectual or developmental disabilities as part of that experience)
OR one year of experience in treating or working with a person who has intellectual disability;
AND is a psychologist, physician, educator with a degree in education from an accredited program, social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, behavior analyst, registered nurse, registered dietician, therapeutic recreation specialist, a licensed/limited-licensed professional counselor,
OR a human services professional with at least a bachelor’s degree in a human services field.

Qualified Mental Health Professional (QMHP) – Individual with specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience)
OR one year of experience in treating or working with a person who has mental illness;
AND is a psychologist, physician, educator with a degree in education from an accredited program, social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, behavior analyst, registered nurse, therapeutic recreation specialist, licensed/limited-licensed professional counselor, licensed or limited licensed marriage and family therapist, a licensed physician’s assistant,
OR a human services professional with at least a bachelor’s degree in a human services field.

Experience:

One year of working with the IDD or SPMI population. Able to meet requirements as a Qualified Intellectual Disability Professional (QIDP) or Qualified Mental Health Professional (QMHP). Proficient computer skills including Office 365.

Other:
Ability to interact with physicians and other healthcare providers in a professional manner. Working knowledge of physical and mental health, chronic health conditions, and behavioral health medications.

SUMMARY OF RESPONSIBILITIES

The Supports Coordinator provides service coordination and/or targeted case management to persons with severe and persistent mental illness or an intellectual or other disability with or without a co-occurring substance use disorder according to a Plan of Service designed to promote recovery or optimize function and community integration. The Supports Coordinator serves as a link between clients’ primary and other healthcare providers to improve coordination of care and facilitate the ongoing flow of information.

The Supports Coordinator advocates for justice, inclusion, and fair opportunity, and links and coordinates services toward the goal of achieving the aims of the Person-Centered Plan. This position supports Antrim/Kalkaska counties.

ESSENTIAL JOB FUNCTIONS
  • Develop, review, deliver and modify as needed, a Person-Centered Plan of Service in collaboration with the client, parent or guardian, designated advocate, residential and day services support workers, and/or clinical professionals.
  • Act as a link between the service provider system including agency staff; the client’s primary care physician, and other community health care and human services providers; and the client, and /or parent or guardian. Assist in the development of natural supports.
