Heartland Home Health, a Gentiva company, is seeking a Full‑Time Physical Therapist to join our Midland, MI team covering the Saginaw territory. This position delivers personalized, meaningful care in the comfort of patients’ homes.

• Deliver skilled physical therapy to patients based on individualized Plans of Treatment developed with the referring physician.
• Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function.
• Adjust treatment plans and techniques as needed to achieve optimal outcomes.
• Collaborate with the interdisciplinary home‑health team—including nurses, occupational therapists, speech therapists, and social workers—to provide coordinated care.
• Educate patients and caregivers on exercises, safety, mobility strategies, and use of assistive devices.
• Document all visits, assessments, and progress notes accurately and in a timely manner.
• Support patients in maximizing independence and improving quality of life regardless of where they live.

• Master’s degree in Physical Therapy from an accredited program.
• Current PT license in Michigan (or eligibility to obtain one).
• CPR certification required.
• Strong communication, assessment, and problem‑solving skills.
• Ability to work independently in a variety of home environments.
• Valid driver’s license, auto insurance, and reliable transportation.
• Preferred: 1+ year of experience in hospital, home health, outpatient rehab, in‑home therapy delivery, geriatric, orthopedic, or neurological rehab, post‑acute care, or chronic condition management; experience working as part of a mobile or interdisciplinary care team.
