SUMMARY OF RESPONSIBILITIES:

Will assist the individual to increase or maintain personal self sufficiency, facilitating goals of community inclusion and participation. Will assist individuals served gain access to participation in recreational, volunteer, and other community-based activities of their choosing. May participate in the interdisciplinary team meetings and person-centered planning process. Assist, prompt, and support individuals served in activities identified in the Individual Plan of Service (IPOS).

Will assist with preserving the health and safety of individuals served to support the most integrated and independent setting. CLS activities will focus on improving independence and enhancing recovery.

QUALIFICATIONS:

Education:

High School diploma or GED preferred.

Experience:

Knowledge of intellectual/developmental disabilities (I/DD) and mental health issues desirable. Ability to communicate effectively with individuals, families, natural supports, and providers. Lived experience with mental illness, intellectual/ developmental disabilities, and/or substance use disorders valued.

Other:
Must be at least 18 years old. Must possess a valid driver's license and acceptable driving record (as determined by the agency). Ability to work a varied schedule including evenings, weekends, and holidays, and occasional overnight trips as needed. Ability to maintain favorable public relations.

ESSENTIAL JOB FUNCTIONS:

1. Provide support and services to individuals served as identified in the IPOS.
2. Adhere to agency policy and procedures for assisting individuals with medication setup and/or delivery.
3. Assist in finding and maintaining volunteer opportunities if desired.
4. Work effectively with providers, families, natural supports, and case managers concerning assessments, treatment planning, and service implementation.
5. Will assist with preserving the health and safety of individuals served to support the most integrated and independent setting.
6. Ability to prepare progress notes, reports, and required documentation according to agency clinical documentation standards.
7. Demonstrates knowledge of universal precautions, standards of practice, and emergency protocol.

NON
- ESSENTIAL JOB FUNCTIONS:

1. Performs other related duties as required.
2. May be requested to be involved in the Board's continuous quality improvement program.
3. May be required to provide own transportation while performing agency-related duties.

Ability to walk, bend, stand, sit, lift up to 50 pounds, with or without assistance, stretch/reach, hear,see, hand/finger dexterity, and drive.
