About COREWELL HEALTH VNA

Our Visiting Nurses Association (VNA) is the Home Care Team serving West Michigan. We are seeking a licensed Occupational Therapist to join our growing team supporting patients across West Michigan, specifically Big Rapids, Reed City, and surrounding areas. This role offers a high level of autonomy, allowing clinicians to practice at the top of their license while delivering care in patients’ homes.

Our therapists maintain manageable caseloads, typically seeing 5–7 patients per day, with thoughtfully designed geographic territories that minimize travel and support work‑life balance. As part of Corewell Health West, our VNA team is committed to delivering exceptional, patient‑centered care across Ottawa County and surrounding communities.

Scope of Work
  • Collaborate with an interdisciplinary team to provide therapeutic services including evaluation, treatment, and education.
  • Monitor the treatment activities of COTAs and aides.
  • Complete evaluations of assigned patients in accordance with established standards of care and practice.
  • Plan and deliver treatment to assigned patients as appropriate in accordance with established standards of care and practice.
  • Complete all documentation of services per department standards.
  • Attend all required safety training programs and describe responsibilities related to general safety, department/service safety, and specific job‑related hazards.
  • Follow hospital exposure control plans and blood borne/airborne pathogen protocols.
  • Demonstrate respect and regard for the dignity of all patients, families, visitors, and employees to ensure a professional, responsible, and courteous environment.
  • Promote effective working relations and work effectively as part of a department/unit team inter‑ and intra‑departmentally to facilitate the department’s/unit’s ability to meet its goals and objectives.
  • Maintain updated license and submit evidence of status to supervisor/manager within two weeks of receipt of new or updated license.
Standard Qualifications
  • Graduate from an accredited OT school.
  • Successful completion of clinical internship.
  • Previous clinical experience/rehabilitation experience preferred.
  • Current State OT license.
  • BLS certification.
  • One year or more experience working in Home Care or VNA (Preferred).
  • Driver’s License – State of Michigan (Required upon hire).
