Summary

Community Health Workers are individuals who have a unique background and skill level from their experience in using services and supports to achieve their personal goals of community membership, independence, health wellness, and productivity. Under immediate supervision of the Clinical Supervisor, the CHW serves as an advocate to individuals we serve; provides information and health support. The CHW performs a wide range of tasks to assist individuals we serve in regaining control over their own lives and over their own recovery process.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Teach and role model the value of every individual's recovery experience.
  • Screens individuals for SDOH that may contribute to disease and/or present as barriers to self-management
  • Coordinates and provides access to chronic disease management
  • Implements wellness and prevention initiative
  • Provides education health conditions and strategies to implement care plan goals, including both clinical and non-clinical needs
  • Accompanies individual to appropriate and supports groups
  • Coordinates and provides access to individual and family supports, including referral to community supports groups
  • Meets regularly with the care team to plan care and discuss cases and provide feedback as needed to promote treatment success
  • Assist individuals we serve in articulating personal goals for recovery.
  • Assist individuals we serve in determining the objectives they need to reach their recovery goals.
  • Attend staff meetings to provide information and training BHH as directed by supervisor
  • Assist individuals we serve in creating a Wellness Recovery Action Plan (WRAP) and/ or relapse prevention plan.
  • Utilize and teach problem solving techniques with individuals and groups.
  • Attend professional advancement opportunities to maintain skills.
  • Other duties as assigned.
