- ** Position Overview
** The Case Manager supports individuals with SMI, SED, and/or I/DD by coordinating services that promote independence, community inclusion, and overall well-being. This role involves advocacy, service linkage, and monitoring to ensure continuity of care and prevent service gaps.
** Persons with lived experience in mental illness, developmental disabilities, or substance use disorders are encouraged to apply.**
- ** Key Responsibilities
*** Develop and implement person-centered service plans
* Link individuals to medical, social, educational, and therapeutic resources
* Coordinate and monitor services to achieve successful outcomes
* Provide advocacy and support to reduce barriers to care
* Participate in emergency on-call rotation
- ** Qualifications
* ** Bachelor's degree in a human services field
* Minimum one year of experience in mental health
* Certification as QMHP, QIDP, and/or CMHP, or Michigan licensure (limited or full)
* Strong organizational, communication, and problem-solving skills
* Ability to work flexible hours and travel within service area
* Valid Michigan driver's license, vehicle insurance, and satisfactory driving record
