** What You'll Do
*** Provide case management and support services for children and youth up to age 21.
* Collaborate with families, schools, and community partners to ensure comprehensive care.
* Implement person-centered plans and monitor progress toward goals.
* Maintain accurate documentation and comply with agency standards.

** Qualifications**
* *
* Education:

** Bachelor's Degree (BA/BS).
* Complete MDHHS Wraparound training.
* ** Have a criminal history screen, including state and local child protection agency registries.**
* ** Be supervised by an individual who meets the criteria as a Child Mental Health Professional (CHMP) who has completed MDHHS required training.**-###
** Skills & Abilities
*** Proficiency in Microsoft Office Suite and standard office technology.
* Strong communication, organization, and time management skills.
* Critical thinking and problem-solving abilities.
* Attention to detail and accuracy in documentation.

*** Emergency Services (E.S.) Certification
* CAFAS Certification
* Wraparound Certification-###
** Other Requirements
*** Valid Michigan driver's license, vehicle insurance, and good driving record.
* Ability to work flexible hours and travel within service area.
* Must pass pre-employment background checks (criminal, driving, education, employment).
