Summary

This position is responsible for following an individual patient care plan to assist patient and/or perform personal care when an individual or family cannot perform the tasks. The aide will care for all types of diagnoses and in many different phases of illness.

Responsibilities
  • Provide hands‑on personal care and demonstrate competency with assigned tasks; follow patient’s plan of care for completion of tasks assigned.
  • Perform simple procedures as an extension of nursing services; assist in ambulation or exercises.
  • Comply with infection prevention and control policies and procedures.
  • Create successful interpersonal relationships with the patient and family; honor patient rights.
  • Be a member of the interdisciplinary team; report changes in the patient’s condition to a registered nurse.
  • Complete pertinent records in compliance with policy and procedures.
  • Safely drive a vehicle to transport residents to and from medical appointments and recreational outings as needed; assist residents to safely enter and exit the vehicle; properly secure residents and wheelchairs; perform routine checks, keep accurate mileage logs, ensure vehicle cleanliness and mechanical soundness; obey all traffic laws and company policies.
  • Maintain clear and professional communication with residents, facility staff, and families.
Other Duties and Responsibilities
  • Promote patient independence and assist or perform personal care (bathing, dressing, shampooing, shaving, oral care, skin care, toileting, changing bed linens, etc.).
  • Accurately perform and document vital signs per care plan; report abnormal findings promptly.
  • Promote skin integrity by keeping skin clean and dry; use pressure‑preventative positioning; observe for skin changes and report to nurse or manager.
  • Use proper positioning techniques when repositioning patient in bed, ambulating, or transferring in & out of bed or chair.
  • Maintain a safe and secure environment during care provision.
  • Follow patient’s plan of care and accurately document care provided and other pertinent information.
  • Demonstrate a positive, courteous, and helpful attitude toward patients/families and customers.
  • Apply organizational policies and procedures; maintain knowledge of equipment use.
  • Perform other duties as assigned.
Certifications and Licences
  • Certified Educated Nurse Aide (CENA) – CNA licensure required within 4 months of hire or transfer.
  • Fingerprinting.
  • Basic Life Support (BLS) – within 1 month of hire or transfer.
  • Chauffeur License.
  • DMV Record Check.
Required Education

High school diploma or equivalent.

Experience, Training and Skills
  • Attend meetings and in‑services; complete at least 12 hours of continued education/in‑service training within a 12‑month period.
  • Communicate effectively verbally and in writing; speak, read, write, and understand English.
  • Relate with all ages and individuals from different cultural and spiritual backgrounds.
  • Work with patients and families dealing with terminal illness, the dying process, and death.
  • Handle multiple tasks; work independently; maintain regular attendance; stay alert.
Physical and Mental Requirements
  • Expose to stressful situations, public contact, trauma, grief, and death.
  • Wear personal protective equipment as required.
  • Move freely about facility with or without an assisted device; perform job functions.
  • Vision and hearing necessary with or without assisted devices.
  • Sit, stand, walk for long periods; perform postural changes such as stooping, kneeling, or crouching.
  • Exposure to blood‑borne pathogens and other potentially infectious materials; complete required testing.
  • Handle repetitive movements; lift/carry up to 21-50 lbs. (0-33% of workday), 20 lbs. (34-66%), and 10 lbs. (67-100%).
