Position Summary

Provides quality resident-directed care and services to residents in a home-like environment which recognizes resident’s individuality, promotes their autonomy, protects their rights, respects their dignity, and offers them freedom of choice. Assists with activities of daily living, administers medications, updates and maintains records and documents related to residents, and assists with light housekeeping, meal service and life enrichment activities.

  • Utilize the resident service plan and resident story to provide personalized and appropriate care for each resident.
  • Assist with personal care, ambulation, transfers, and positioning as needed while maintaining the privacy, comfort and dignity of each resident.
  • Respond promptly to the notification system signaled by the resident.
  • Administer medications as directed by the physician.
  • Monitor, document and report changes in the resident’s condition to the clinical support team.
  • Monitor, intervene and report unsafe conditions and document incidents and or accidents when they occur.
  • Report any actual or suspected waste, fraud or abuse to the Director and/or designee immediately.
  • Perform light housekeeping duties and assist with serving meals, snacks and beverages.
  • Communicate openly and honestly at all levels and work as a cohesive team to create and maintain an environment in which everyone can thrive.
  • Engages residents in unplanned and planned activities supporting resident’s psychosocial well-being.
  • Provide excellent customer service, support and encouragement to residents, families, coworkers and other customers through the Resthaven Way culture.
  • Attend and participate in orientation programs, on-going training and educational classes.
  • Follow established safety precautions, infection control protocols and maintain privacy and confidentiality as required by Resthaven’s Corporate Compliance program.
  • Follow established smoking regulations and report violations.
  • Completes incident reports and related paperwork a necessary.
  • Perform other related duties as assigned and/or requested by the resident.
Education, Training, Experience
  • High school graduate or successful completion of GED.
Physical Requirements

This role is active and hands-on. You must be able to:

  • Stand, walk, bend, lift, push, pull, and assist residents throughout your shift
  • Lift up to 50 pounds
  • Use mechanical lifts and clinical equipment safely
  • Maintain visual acuity and physical stamina required for resident care
