At Health Source Saginaw, we are seeking a compassionate Social Worker to join our Medical Rehabilitation Center program. This role offers an opportunity to make a meaningful difference in the lives of patients and their loved ones while working on an as-needed basis.

The Social Worker will perform the full range of assignments including but not limited to:

• Complete psychosocial admission assessments, including evaluation of psychosocial and emotional functioning, events precipitating admission, identification of patient needs, limitations, and strengths.
• Coordinate the development and implementation of individualized treatment plans in consultation with the patient and other treatment team members; revise treatment plans accordingly and ensure the awareness of insurance coverage and estimated length of stay are appropriate for patient needs.
• Maintain timely communication with patient’s family/significant relations and be a liaison between family, facility, and community resources.
• Provide intervention services including crisis intervention, individual, and family therapies in relation to wellbeing related to illness, disability, and hospitalization. Identify/make recommendations for alternate treatment methods as necessary.
• Coordinate care conferences, discharge planning, and family training with the interdisciplinary team and community resources. Ensure all equipment or supplies are accurately ordered and delivered for proper discharge.
• Assess the need for staff development programs in relation to psychosocial needs of the rehabilitation patients and their support systems.
• Maintain professional knowledge through participation with continuing education to provide care appropriate to the age of the patients served in the therapy setting.
• Complete discharge follow-up calls to ensure patient and family have a successful discharge back to community setting.
• Assist the nursing unit to bridge communication of patient needs and support, while also supporting patient intake and admitting process.
• Schedule and organize transportation to/from outside appointments ensuring family training has been completed if family is scheduled to transport.
• Perform additional assignments as requested by the Medical Rehabilitation Center Executive and/or Nurse Manager or by changes in current Federal, State or regulatory agency requirements/guidelines.

• Completed Bachelor’s or Master's Degree from an Accredited School of Social Work (BSW/MSW).
• Must be licensed, or eligible for licensure, by the State of Michigan.
  • If practicing under a limited license, full licensure is expected within two years unless an extension is approved by the direct supervisor.
  • Must remain knowledgeable of applicable regulations and ensure appropriate supervision coverage when practicing under a limited license.
• Current BLS/CPR certification. Onsite training is available if needed.
