Position: Community Health Worker (HYBRID) - Tawas
Location: Tawas City

Summary

This is a grant funded-temporary position with anticipated funding for one year. Continued employment beyond that time frame in this position is dependent upon grant funding availability.

The Community Health Worker (CHW) serves as a frontline liaison between My Michigan Health and the communities we serve. This role is responsible for conducting outreach and care coordination activities to connect patients with appropriate health care, community, and social service resources that address social determinants of health (SDOH) barriers and improve overall health outcomes. This person will be a key member of ambulatory patient-centered health teams and community collaborations and will work closely with the Community Health and Population Health teams.

Responsibilities
  • (50%)
    * Manage Community Health work queue referrals for assigned geographies. Conduct patient outreach to address identified social determinants of health (SDOH) needs. Assist patients and families in addressing health disparities by connecting them with appropriate community resources to remove SDOH barriers.
  • (25%)
    * Promote wellness and health equity by providing locally relevant, culturally appropriate education, resources, and support to patients and community members. Assist patients with chronic disease prevention and management.
  • (25%)
    * Maintain current knowledge of SDOH best practices, CMS requirements, community resources, and referral processes. Utilize standardized Epic documentation workflows and templates to ensure accurate, compliant, and timely documentation of interventions, outcomes, and resource connections.
OTHER DUTIES AND RESPONSIBILITIES
  • Support care team needs in provider offices as appropriate to meet payor metrics and billing guidelines.
  • Establish/maintain positive relationships with community partners.
  • Collaborate with Community Health team if/when a need for a new community relationship(s) is identified.
  • Advocate for patients when needed.
  • All other duties as assigned.

Certifications and Licensures
  • CHWCore Comp
    - Community Health Worker Core Comp
  • Required Equivalent Experience
    - Community Health Worker Core Competency certification is required within 6 months of hire or transfer. Cardiovascular Health (CVH) and Diabetes Micro credential course completion within 6 months of hire or transfer.

High School Diploma or GED

SKILLS:

Experience in a health system, public health sector, or community benefit organization.

Must have strong written and oral communication skills, including preparation and report presentation.

Knowledge of personal computers, PC applications, MS Word, Excel, PowerPoint and Project.

Must be self?motivated, strong problem solver, and detail?oriented.

Effectively communicate with community members, partners and stakeholders.

Communicating health information using culturally appropriate terms and concepts.

Capability to navigate complex social/health issues.

PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS

Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death.

Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position.

Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description.

Overall vision and hearing is necessary with or without assisted device(s).

Frequently required to sit/stand/walk for long periods of time.

May require frequent postural changes such as stooping, kneeling or crouching.

Some exposure to blood borne pathogens and other potentially infectious material.

Must follow My Michigan Health blood borne pathogen and TB testing as required.

Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake.

Overall dexterity is required including handling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis. Physical Demand Level: Sedentary. Must be able to occasionally (0?33% of the workday) lift or carry 0?10 lbs
