SUMMARY

The Advanced Practice Provider (APP) provides comprehensive, patient-centered primary care services to patients of the Gun Lake Tribe Health & Human Services (HHS) Clinic. Working collaboratively with physicians, behavioral health providers, nursing, pharmacy, community health, traditional healing, and other interdisciplinary team members, the APP delivers high-quality, evidence-based care that supports the physical, emotional, behavioral, spiritual, and social well-being of patients across the lifespan.

Guided by HHS’s CEDAR values – Connected, Engaged, Deep-Rooted, Aware, and Responsive – the APP builds trusting relationships, honors Anishinaabe values and traditions, practices trauma-informed care, and partners with patients to improve health outcomes while promoting wellness, prevention, and continuity of care.

DUTIES AND RESPONSIBILITIES
  • Provides primary medical care to patients with various medical conditions, serves patients across their lifespan.
  • Analyzes and interprets patients’ histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses.
  • Orders and interprets laboratory tests as needed to diagnose and treat patients.
  • Develops and communicates diagnosis and treatment plans to patients and, when appropriate, to family and other involved caregivers.
  • Prescribes medications, prescribes treatments or therapies, makes referrals, schedules follow up and ensures ongoing care.
  • Effectively treats and manages patient care in a holistic manner and uses an integrated treatment approach including making referrals to and collaborating with other HHS programs to coordinate comprehensive care plans that improve patient outcomes.
  • Participates in interdisciplinary case reviews and care coordination activities.
  • Delivers comprehensive, high-quality care to diverse patient populations, including pediatric and geriatric patients, while addressing a broad spectrum of women's and men's health needs through evidence-based practice and patient-centered care.
  • Educates patients about self-management of acute and chronic illnesses, tailoring instructions to patients’ circumstances.
  • Promotes preventative care, chronic disease management, and health education through shared decision-making.
  • Serves as a clinical resource and mentor to nursing staff and other healthcare professionals.
  • Participates in population health initiatives and quality improvement efforts and activities of the medical clinic.
  • Protects patients and employees by developing and interpreting infection-control policies and protocols; enforcing medication administration, storage procedures, and controlled substance regulations.
  • Produces complete and accurate medical records for each patient encounter using the Electronic Medical Record system.
  • Maintains and protects patient confidentiality in accordance with HIPAA.
  • Participates in peer reviews.
  • Participates in professional capacity at off-site special events as assigned.
