Case Manager – Bronson Battle Creek

Variable shifts including weekends and holidays. May work inpatient and emergency department.

Responsibilities
  • Move patients from admission through discharge without disruption to their care, including assessment, planning, implementation, coordination, monitoring, and evaluation.
  • Ensure appropriate, cost‑effective care based on patient needs and hospital capabilities.
  • Serve as an advanced clinical resource to patients, families, and staff.
  • Collaborate with the interdisciplinary team to provide a continuum of comprehensive care.
  • Monitor outcomes and participate in continuous improvement initiatives.
  • Perform early assessment and identify patients at risk for post‑hospitalization care and services.
  • Conduct further assessments/interviews and psychosocial evaluations as needed.
  • Initiate, facilitate, and revise patient transition care plans with care coordination team members.
  • Manage and monitor patient progress and document per procedure.
  • Maintain ongoing assessment and contact with patients during care.
  • Round daily on all assigned patients and identify readmissions, reasons, and required interventions.
  • Ensure smooth, efficient transition of care between services and drive multidisciplinary team rounds.
  • Document clear and specific transitional planning reflecting patient care needs and choices.
  • Enact transitional plan to move patients along the care continuum.
  • Work with community resources to identify and allocate post‑discharge needs, including referrals to appropriate services.
  • Act as liaison among patients, physicians, ancillary staff, and community services throughout the patient experience.
  • Develop, implement, coordinate, and communicate care plans from acute phase through transition out of acute care.
  • Build and maintain relationships with physicians, nursing team and leaders to ensure quality of care.
  • Coordinate care using Pathways or Plan of Care and contribute to ongoing development and revision.
  • Participate actively in assigned groups and committees.
  • Ensure appropriate use of community and outpatient resources to support post‑discharge needs.
  • Manage and coordinate discharge plans to achieve appropriate length of stay and favorable clinical/financial outcomes.
  • Evaluate outcomes related to the case management process, including LOS, readmission, patient satisfaction, and financial variances.
  • Work with multidisciplinary team and providers to create standard care plans for patients and review behavioral contracts as needed.
  • Assess eligibility of uninsured and underinsured patients for federal, state and community programs and assist with billing paperwork.
  • Function as a liaison to community programs and maintain contact with appropriate resources to meet patient needs.
Qualifications
  • Licensed Registered Nurse in good standing with the State of Michigan.
  • BSN upon hire (required);
    Master’s degree preferred.
  • At least 3 years of experience in an acute care hospital setting.
  • Case Management Certification preferred.
  • Proficient in word processing, spreadsheets, keyboarding, presentation programs, and related software.
  • Ability to handle multiple priorities in a stressful environment.
  • Effective verbal and written communication with all healthcare levels.
  • Ability to communicate clearly, collaboratively, and supportively to patients and families.
  • Strong critical thinking, decision‑making, and problem‑solving skills.
  • Negotiation, influence, and timely follow‑up abilities.
  • Ability to assess, prioritize, plan, organize, monitor, and evaluate patient needs and skill level.
  • Independent self‑motivation and conflict‑resolution skills.
  • Physical ability to walk, stand, stoop, bend, sit, lift light objects, and operate a personal computer.
Schedule

Part‑time, variable shifts, 10 weekly hours.
