REGISTERED NURSE – CASE MANAGER – HOME CARE

Aspirus Iron River Hospital in Iron River, MI seeks a registered nurse to serve as a Case Manager on our MNA Home Care team.

Responsibilities include assessment, planning, implementation, coordination, monitoring, and evaluation of nursing care for home health and hospice patients of all ages and their families, across all settings. The candidate develops care plans in partnership with Home Health & Hospice employees and volunteers, Aspirus Inc. entities, and community partners.

HOURS:

Full Time, 1.0 FTE (80 hours per pay period), Day Shift, On‑call rotation.

• Knowledge of patient care standards, normally acquired through completion of a Diploma, Associate Degree or Bachelor’s Degree in Nursing.
• Active Wisconsin or Michigan nursing license and current CPR certification.
• Previous experience working in home health or hospice; at least five years of broad-based nursing experience, with additional training or experience with the terminally ill considered a benefit.
• Current BLS certification or completion within 90 days of hire.
• Valid Wisconsin or Michigan driver’s license.
• Ability to travel to patient/client homes to provide care.
• Auto liability insurance with minimum $100,000/$300,000 coverage required.
• Respect for patient and family rights, delivery of excellent home health and hospice care, and promotion of quality of life.
