Overview:
Matrix Medical Network pioneered the first national in‑home clinical network. It provides comprehensive health assessments to people nationwide, delivering personalized care that improves health, lowers costs, and empowers patients to live their best lives. The network offers clinicians the ability to meet at members’ homes or skilled nursing facilities and provides opportunities to impact the health and well‑being of older adults and other at‑risk individuals.
- Conduct adult/geriatric assessments, including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow‑up, and clear documentation per Matrix guidelines.
- Administer and order point‑of‑care and lab‑based testing as needed.
- Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Collaborate with the Primary Care Physician on patient education and follow‑up as necessary.
- Provide services at the member’s place of residence, which may be in their homes or skilled nursing facilities.
- Flexibility:
Visits are scheduled based on your availability. Assessments run Sunday through Saturday, 8 am–7 pm, and you can work during day, evening, or weekend shifts. - Competitive Pay:
Earn income based on the number of assessments completed. Per‑assessment rates range from $90‑$115, depending on state, with monthly totals of $1,800‑$11,500 for 20‑100 visits. - Benefits:
Mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits. - Education and Support:
Comprehensive onboarding training, a preceptor, 1:1 feedback, and free access to continuing education. - Additional Licensure:
Support for obtaining state licensure and credentialing in neighboring states if needed. - Full‑time Opportunities: PRN providers may transition to full‑time roles with a sign‑on bonus.
- Master’s degree or equivalent experience and satisfactory completion of NP licensure.
- Current Nurse Practitioner licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certification from AANP, AACN, or ANCC in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialties.
- Current BLS, ACLS, or CPR certification.
- Preferred: 1 year of NP experience (new graduates encouraged to apply).
- Valid state driver’s license and automobile insurance coverage for the state of residence.
- Strong computer skills and familiarity with employee/medical record software.
- Excellent verbal and written communication skills.
Liability coverage requirements
:
Bodily Injury: $100,000 per person / $300,000 per incident. Property Damage: $50,000. Collision and comprehensive deductibles: $1,000 or less.
