Home Care and Hospice Certified Nurse Aide (HCHCNA) – PRN

Assists the professional and technical nursing staff in performing standardized duties in caring for patients in their place of residence. The patient population includes caring for injured, ill, and end‑of‑life patients based upon the age of the individual served. The HCHCNA administers basic procedures designed to provide total patient care and documents the care given and the patient’s response.

Home Health Coordinator or RN Case Manager

• Graduated from an approved secondary education program (high school) or possess an equivalent high school diploma.
• Successfully completed a formal nurse aide program and maintains current certification.
• Successfully completed a state‑approved competency evaluation within four months of the date of hire.
• Can express ideas logically.
• Understand the Hospice philosophy.
• Complete informal training regarding the Hospice philosophy provided by the social worker, Director of Home Services, or an RN Case Manager.
• Demonstrate personal acceptance of responsibility and accountability for self and team members.
• Promote positive interpersonal relationships with staff members, administrators, physicians, and the public.
• Accept assignment duties within knowledge, experience, and educational training.
• Maintain confidentiality concerning patient/family, staff members, administration, and physicians.
• CPR certified every two years according to the Michigan Heart Association guidelines.
• Willing to work flexible hours, including hours outside the normal business day, which may vary above or below 40 hours per week.
• Demonstrate a daily commitment to the Standards of Behavior and Core Values of BCMH.
• Maintain a valid driver’s license, clean driving record, and vehicle insurance.
• Ability to read, write, and verbally report clinical information to patients, representatives, caregivers, and other HHA staff.
• Recognize and report changes in skin condition, including pressure ulcers.

• Perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing.
• Deal effectively with stressful situations.
• Maintain a professional image of the company by speaking positively in front of patients, the public, co‑workers, or health professionals outside of Baraga County Home Care and Hospice. Be polite and courteous.
• Maintain dress code, wearing name tag, and remain clean and neat at all times.
• Demonstrate ability to maintain work standards by keeping work area clean and orderly and by resolving minor employee conflicts independently.
• Follow direction from the Director of Home Services and/or an RN Case Manager, understand the role of the organization and other departmental roles, and respond to constructive criticism.
• Assist as necessary during busy times.
• Prioritize and complete work timely and accurately. Report to work on time and observe time frames for lunches and breaks.
• Work evenings, weekends, and holidays according to job category requirements and keep overtime to a minimum.
• Maintain client/family confidentiality at all times.
• Locate Bill of Rights, advance directives, disaster manual, and policy and procedure manual when needed.
• Complete all required documentation timely and correctly with minimal errors.
• Identify and report victims of abuse.
• Be aware of community resources and report possible needs as necessary.
• Participate with alternate caregivers as indicated and participate in interdisciplinary group meetings (IDG) with appropriate disciplines.
• Complete appropriate preventative, therapeutic, and rehabilitative nursing procedures in accordance with the organization’s policies, procedures, and competency standards.
• Perform satisfactorily clean techniques.
• Observe patient’s environment and report safety concerns to the RN. Identify symptoms of abuse and follow reporting protocols.
• Use gait belt when patient requires assistance with transferring or ambulating.
• Adhere to universal precautions at all times.
• Obtain and transport supplies to the patient’s home in appropriate conditions as directed by the RN.
