Looking for meaningful nursing work with flexibility?The Care Team Hospice is seeking a compassionate Part-Time Registered Nurse to provide exceptional end-of-life care to patients in facility settings and private homes. Join a supportive team environment where you can make a meaningful impact while delivering compassionate, patient-centered hospice care to patients and families during life’s most important moments.
Schedule, Hours & Territory- Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
- Territory includes West Branch, MI, and surrounding areas within approximately a one-hour radius based on patient census and branch needs.
- Strong interdisciplinary team collaboration
- Supportive clinical leadership and resources
- Clear expectations and consistent workflows
- Environment focused on quality, compliance, and patient-centered care
- Conduct comprehensive patient assessments and develop individualized plans of care
- Provide skilled nursing care in accordance with hospice standards and physician orders
- Coordinate care with physicians, caregivers, and interdisciplinary team members
- Educate patients and families on symptom management, disease progression, and end-of-life care
- Document assessments, interventions, and care plans accurately and on time in the EMR HCHB (Homecare Homebase)
- Participate in interdisciplinary team meetings and care planning
- Support after-hours patient needs as part of an on-call rotation when applicable
- Active Registered Nurse (RN) license
- Hospice or home health experience preferred
- Strong clinical assessment and care coordination skills
- Ability to work independently in the field
- Strong communication and documentation skills
- Experience using HCHB (Homecare Homebase) preferred
- Active driver’s license
- Ability to pass a criminal background check
- Ability to pass a Motor Vehicle Record (MVR) check
Current TB testing in accordance with agency policy - Current CPR certification as required for the role
