The Licensed Practical Nurse (LPN) works under the direction of the Team Leader. The LPN gathers assessment data, monitors, follows the plan of care and promotes an optimal level of health. The LPN provides medication administration, treatments, and supervision of CENAs performing ADLs. The LPN collaborates with the Care Coordinators and other team members to ensure communication and timely updates on residents’ changes in conditions.
Key Responsibilities- Gathers assessment data, monitors comprehensive physiological, psychological, sociocultural, and spiritual needs of patients and significant others under the supervision of the Team Leader and reports findings to the Team Leader.
- Provides competent care consistent with the nursing process, the written plan of care, physician’s orders, and in accordance with skilled nursing facility policies and procedures.
- Participates in admission and discharge planning, in collaboration with the health care team, at the time of admission and continues through discharge.
- Provides patient and family education to meet their specific needs as outlined in the plan of care and delegated by the Team Leader.
- Documents care consistent with guidelines.
- Performs other duties consistent with the purpose of the job as directed.
- Graduate of an accredited school of nursing.
- License to practice practical nursing in the State of Michigan.
