Location: West Branch
Summary
The registered nurse will perform direct patient/resident care and delegate to patient care technicians to provide care and comfort to residents. The RN will maintain knowledge and compliance with all local, state, and federal regulations and guidelines relating to facility care, and possess basic medical/surgical knowledge to care for general surgical and orthopedic surgical patients.
Responsibilities- Regularly assess patient’s needs and provide skilled nursing services per physician-authorized plan of care and accepted standards of nursing practice. (30%)
- Provide direct resident care, including medication administration, treatments, call lights, assisting with transfers, responding to alarms, etc. (30%)
- Evaluate effectiveness of treatment plan and suggest or participate in updates. (20%)
- Process physician/provider orders and perform ordered care accurately, completely and in a timely manner. (10%)
- Follow up regarding medication changes and order clarifications. (10%)
- Demonstrate the ability to interpret information and report pertinent data to the physician, including pertinent clinical information.
- Assess learning needs considering readiness, ability, preferences, and cultural/spiritual variables.
- Provide education to meet the individual needs and ability to learn.
- Participate in care conferences as assigned.
- Develop and maintain good working rapport with interdisciplinary personnel to meet the needs of each patient.
- Coordinate care provided by patient care technicians.
- Demonstrate the ability to organize and prioritize the needs of groups of patients or an individual patient.
- Demonstrate a general ability to manage assignments and perform routine duties on the unit.
- Instruct other team members when appropriate, explaining goals and rationale for individualized care.
- Implement clinical and technical aspects of care in accordance with established policies and procedures.
- Demonstrate understanding of clinical pathways and utilize appropriately.
- Remain calm and efficient when working in stressful situations.
- Document in an accurate, clear, and concise way and include pertinent clinical data.
- Share information and report significant changes and/or abnormal findings to appropriate team members.
- Participate in regulatory surveys.
- Participate in performance improvement activities.
- Assist with orientation and ongoing competencies.
- Provide a safe and secure environment for patient care.
- Attend and participate in professional association activities and programs to assure being current on regulations, guidelines, and professional standards.
- Remain current on new developments by reading professional journals and attending seminars.
- Demonstrate understanding of the role in emergency preparedness procedures.
- Set up and assist with central line and chest tube insertion and care for these tubes according to policy and procedure.
- Insert cantor tubes, nasogastric tubes, and feeding tubes per physician order, following proper procedure.
- Insert and provide care for IVs.
- Provide routine care and instruction of suprapubic (SP) catheters and surgical drains following physician order.
- Provide proper care for drains and removal of same.
- Remove sutures and skin clips, provide routine ostomy care and irrigate and pack wounds following physician order, procedure and care plan.
- Do accurate Central Venous Pressure (CVP) reading, according to physician order and following proper procedure.
- Set up, instruct and monitor use of PCA.
- Obtain doppler pulses following proper procedure.
- Provide routine care of gastrostomy/jejunostomy tubes according to procedure and physician orders.
- Assist in heparin therapy protocol per procedures.
- Apply basic traction, splints, braces, and Continuous Passive motion (CPM) machines following procedure and physician order.
- Cut and trim casts, remove wound drains following physician order and procedure.
- Maintain total hip and total knee precaution measures and instruct patients.
- Other duties as assigned.
- ACLS – Advanced Cardiac Life Support (Required within 12 months of hire or transfer; Effective 5/8/24; current employees have 12 months to obtain the certification)
