Under general supervision of the Director of Nursing, the MDS (Minimum Data Set Assessment) nurse performs MDS assessments on patients, completes insurance certifications and re-certifications, and manages responses for insurance medical record audits. The role is critical to ensuring accurate MDS data, optimizing reimbursement, complying with regulations, and maintaining facility 5‑Star ratings and CMS Quality measures.
Responsibilities- Lead role in managing/determining the MDS process
- Determination of the most optimal ARD Assessment Dates
- Ensuring timely completion
- Monitoring and auditing for MDS accuracy
- Completes Minimum Data Set Assessments on short‑stay patients and residents
- Tracks Priority Health Dashboard Data and Outcomes
- Processes MPRO and KePRO audits and appeals
- Completes chart review and submits appeals for all insurance audits
- Completes insurance certifications and re‑certifications
- Issues NOMNC and ABN forms to patients
- Attends Care Conference Meetings
- Attends Discharge Planning Meetings
- Participates in nursing stand‑by coverage and may be called upon to work as a charge nurse
- Other duties as assigned
- Collects comprehensive data pertinent to the resident’s health or the situation to accurately complete assessments
- Assesses the resident’s physical and psychosocial needs
- Analyzes assessment data to identify appropriate resident needs
- Documents nursing history
- Reviews vital signs to assess condition of resident and detect deviations from normal
- Analyzes assessment data to determine patient problems or issues
- Uses the assessment data to identify resident problems
- Confirms problems with resident and family when appropriate
- Documents the expected outcomes and plan of care related to the problems in the medical record
- Identifies expected outcomes for an individualized plan
- Utilizes resident/family input when defining outcomes
- Incorporates resident values, ethical and cultural considerations
- Coordinates discharge planning for outcomes not achieved utilizing the continuum of care available
- Develops a plan that prescribes interventions to attain expected outcomes
- Incorporates Clinical Practice Guidelines into the plan when appropriate
- Develops individualized interventions with resident input when appropriate
- Considers data from other interdisciplinary team members when developing the plan of care
- Implements the identified plan
- Implements appropriate interventions
- Administers prescribed medication and treatment in accordance with approved nursing techniques and policies
- Utilizes evidence‑based interventions specific to the resident’s plan
- Maintains awareness of comfort and safety needs of the resident
- Initiates a resident education plan according to the individualized needs and coordinates implementation
- Manages the care plan by organizing and delegating interventions; evaluates effectiveness of care given by self and others
- Evaluates progress toward attainment of the expected outcomes
- Evaluates effectiveness of planned interventions and resident response toward achievement of outcomes
- Documents resident response toward expected outcomes
- Associate’s degree, Diploma, or Bachelor of Science from an accredited nursing program prior to employment
- Licensed Registered Nurse in good standing with the State of Michigan
- Successful completion of a clinical orientation program
- MDS certification required within one year of hire
- BLS certification required by completion of core orientation
- MDS experience preferred
- Experience in Long‑Term Care, Rehab, or general Geriatrics settings preferred
- Excellent communication, interpersonal, and organizational skills
- Willing to accept a high level of responsibility and demonstrate accountability
- Ability to prioritize using critical thinking skills
- Willingness to seek additional information from a variety of sources
- Proficient with word processing, spreadsheets, keyboard skills, presentation programs, and other relevant software
- Highly organized and able to work efficiently with a high level of accuracy
The role requires above‑average strength and/or stamina to regularly lift weights up to 100 pounds, and occasionally assist with heavier tasks or equivalent effort in pushing, pulling, or handling material, equipment, and other objects. Work may involve reaching, climbing, maintaining balance, or manipulating heavy objects.
