The Care Team Hospice is seeking a compassionate Hospice RN Case Manager in Standish, MI to coordinate patient care, develop individualized plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity, and quality of life at the end of life.
This on-site role offers Monday–Friday 8:00 AM – 4:30 PM with on-call opportunities for additional compensation.
