Home Health Registered Nurse (RN) – Baylor Shift

You’ll support patient recovery and enable them to stay at home, allowing them to spend quality time with loved ones and continue their daily activities. You will educate patients and families on disease management and provide case management and clinical oversight to the interdisciplinary team.

• Assist patients in recovery and promote health and wellness at home.

• Registered Nurse license in the state of practice.
• Minimum two years of experience as an RN; home‑health experience preferred.
• Valid driver’s license and a clean driving record.
