At Health Source Saginaw, we are seeking an MDS Registered Nurse for our Long Term Care program. In this role, you will oversee MDS (Minimum Data Set) assessments, participate in care planning and discharge planning activities, and collaborate with clinical and administrative teams to support quality resident care.
As an MDS Registered Nurse, you will coordinate and manage the resident assessment process to ensure accurate clinical documentation, regulatory compliance, and appropriate reimbursement. Through your work, you will help residents achieve their best possible outcomes while supporting staff, patients, and residents to Rise to Be Your Best.
What You’ll Do:
The MDS Registered Nurse will perform the full range of assignments for patients/residents including but not limited to:
- Coordinate and oversee the MDS assessment process to ensure accurate patient/resident assessments, care planning, and regulatory compliance.
- Complete and manage MDS assessments in accordance with federal and state guidelines and facility policies.
- Collaborate with nursing, therapy, social services, physicians, and other interdisciplinary team members to support resident care and reimbursement accuracy.
- Review clinical documentation to ensure assessments accurately reflect resident needs and services provided.
- Participate in care plan development and interdisciplinary meetings to support positive resident outcomes.
- Monitor Medicare, managed care, and reimbursement requirements to ensure timely and accurate submissions.
- Support discharge planning efforts by collaborating with the care team to promote safe transitions and reduce avoidable readmissions.
- Provide education and guidance to staff regarding documentation and assessment requirements.
- Assist with quality improvement initiatives, audits, and regulatory readiness activities.
- Maintain accurate records and communication with payers, residents, families, and the interdisciplinary team.
What We’re Looking For:
REQUIRED :
- Graduate from an accredited school of nursing and holding an unrestricted current license to practice nursing in the State of Michigan as a Registered Nurse (RN).
- High level of clinical and technical expertise in Utilization Management, MDS, RAI, third-party reimbursement, discharge planning and working knowledge of diagnoses.
- Resident Assessment Coordinator (RAC) certification or willingness to obtain within 12 months of hire.
- 1-2 years of experience in MDS, RAI, case management and utilization review concepts.
PREFERRED :
- Bachelor's degree in Nursing, Healthcare Administration, or a related field.
- Previous experience in long-term care, skilled nursing, or rehabilitation settings.
