Licensed Practical Nurse

Ciena Healthcare is seeking a Licensed Practical Nurse to provide resident-centered care in our skilled nursing and rehabilitation centers. In this role, you will deliver direct bedside care, administer medications and treatments, monitor resident conditions, and accurately document clinical status. You'll collaborate closely with RNs, physicians, and interdisciplinary team members to support post-acute recovery, memory care, and long-term residents. We emphasize compassionate communication with residents and families, adherence to care plans and regulatory standards, and participation in ongoing training, education, and professional growth opportunities.

Responsibilities

• Provide direct bedside nursing care to residents in skilled nursing and long-term care settings
• Administer medications and treatments per physician orders and facility protocols
• Monitor, document, and report changes in resident conditions to RNs and providers
• Assist with admissions, transfers, and discharge planning documentation
• Follow individualized care plans and participate in care conferences
• Maintain accurate electronic and paper clinical records per regulatory standards
• Collaborate with interdisciplinary team members to support recovery and quality of life
• Educate residents and families on care, medications, and health routines
• Support infection prevention, safety, and quality improvement initiatives
• Mentor and support CNAs and other support staff in daily resident care
