Location: Deckerville
Matrix Medical Network is recruiting Nurse Practitioners to perform in-home comprehensive health assessments across the United States. Each visit lasts up to an hour and focuses on medical history, medications, and social factors to identify care gaps and improve outcomes.
Providers work with physicians, case managers, and other staff, traveling to members’ homes or facilities. Strong NP credentials and state licensure are required, with support for licensure expansion as needed.
