• Provide individualized nursing care to our residents, including medication administration, wound care, and treatment plans

• Develop and implement comprehensive care plans in collaboration with the interdisciplinary team

• Conduct initial and ongoing assessments to identify and address resident needs

• Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination

• Maintain accurate and up-to-date records of resident care and progress

• Collaborate with other team members to ensure a supportive and respectful work environment

• Registered Nurse license in the State of Wisconsin

• Current CPR certification

• Strong clinical skills and knowledge of Skilled Nursing best practices

• Excellent communication and interpersonal skills

• Ability to work effectively in a fast-paced environment
