MDS Nurse

Ciena Healthcare seeks an MDS Nurse LPN to coordinate accurate, timely resident assessments in a skilled nursing and rehab setting. In this resident-centered role, you will complete MDS/RAI assessments, develop and update individualized care plans, and collaborate closely with the interdisciplinary team to ensure regulatory compliance and strong clinical outcomes. Working in our fast-paced, 24/7 care environment, you'll use EHR systems, support survey readiness, and contribute to quality improvement while advancing your skills through ongoing education and growth opportunities.

Responsibilities

• Complete and coordinate timely, accurate MDS assessments and related documentation.
• Collaborate with interdisciplinary team to develop, update, and review resident care plans.
• Monitor resident status and ensure assessments reflect clinical needs and regulatory requirements.
• Use EHR systems to document assessments, care plans, and supporting clinical data.
• Support quality improvement, survey readiness, and compliance with state and federal guidelines.
• Educate staff on documentation standards that support accurate MDS data and care planning.
• Communicate with residents and families regarding care goals and care plan updates.
• Participate in interdisciplinary meetings and case reviews to optimize resident outcomes.
