Responsibilities- Initial and ongoing assessment of the patient’s physical, functional, psychosocial and environmental needs
- Implement the individualized plan of care and revise as needed
- Compliance with Advisa Care Policies and Procedures, Medicare compliance
- Consult and educate the patient/family on disease process, self‑care techniques, end‑of‑life care, and ethical issues
- Supervise paraprofessionals providing services to the patient
- Prepare clinical and progress notes, participate in IDG conferences
- Participate in Home Health and Hospice performance improvement program
- Maintain high level of customer satisfaction with clients, families and caregivers
- Communicate information using current processes and technology within the organization
- Current Michigan license in good standing
- Minimum 1 year home health and hospice experience
- Empathetic approach to patients and caregivers
- Participate in field visits daily, weekly office meetings as indicated
- Ability to adapt to patient’s status and needs
